Bupivacaine is an amide local anesthetic supplied in multiple formulations for regional and local anesthesia. This product is used to produce local or regional anesthesia or analgesia for surgical procedures, dental or oral procedures, and obstetric anesthesia. The appropriate concentrations and presentations vary by block type, and not all blocks are indicated for use with bupivacaine due to clinically significant toxicity risks. Onset is moderately rapid and the duration is longer than that of many other local anesthetics, with analgesia sometimes persisting after sensory return. Administration requires trained clinicians prepared to manage dose-related toxicity, with resuscitation equipment and medications readily available. Do not use solutions with preservatives for neuraxial or spinal anesthesia, and avoid mixing bupivacaine with other local anesthetics. Visually inspect vials for particulates and discoloration before use, and deliver doses incrementally while monitoring cardiovascular and respiratory status after each injection to detect inadvertent intravascular or intrathecal administration. Known adverse events include CNS toxicity (ranging from restlessness and tremors to seizures and coma) and cardiovascular effects such as conduction abnormalities, hypotension, arrhythmias, and cardiac arrest; methemoglobinemia and hypersensitivity reactions may also occur.
